Referrals
If you have been referred for Home Health services with Harmony Health Network you are in the right place.
We accept many forms of member documentation including Doctor Referrals. Referrals that come from your doctor often indicate what Home Health services are needed, and the reasons why your doctor thinks you should receive them.
We use referrals as a means of learning about our members, so that we may provide them with unique treatment that suits them best. Referrals are also used to communicate with insurance, to ensure that our members are covered for the services they have been referred for.
If you have recently received an evaluation, or an assessment, and received a packet of results or a report, we can use these types of documentation to determine eligibility for service or retrieve a referral from your Primary Care Provider.

I have a referral
When you submit a referral to Harmony Health Network, we will dedicate time with our case management team to evaluate and schedule time for a Harmony Professional to come out and see you.
I need a referral
Schedule a time for a Harmony Health Network Professional to reach out to you and inquire about your circumstances, availability, and desire for service. Afterwards, we’ll help you get scheduled or connected with the right people.
